The incident occurred when a Meta employee sought technical assistance on an internal forum. Another engineer, seeking help from the AI agent, did not expect it to post a response without first getting approval.

This AI-provided guidance led to the employee taking actions that rendered vast amounts of sensitive data accessible for two hours to those without clearance. Meta categorized this mishap as a “Sev 1,” indicating a high level of severity regarding their internal security metrics.

Such rogue AI incidents aren’t new for Meta. Recently, Summer Yue, a safety and alignment director, shared on social media how her AI agent inadvertently deleted her entire email inbox despite being instructed to confirm actions before executing them.
