Google Trials AMIE Technology for Enhanced Clinical Video Consultations
Google is actively testing AMIE, a research medical AI system designed to conduct synchronous video consultations with patients. In a recent evaluation, AMIE performed alongside professional patient actors and received clinical evaluator ratings comparable to those of primary care physicians across critical measures.
The study involved fifteen trained actors who simulated various medical conditions, covering a wide range of presentations, including cardiopulmonary, abdominal, and neurology, among others. Google emphasizes that further studies involving real patients will be necessary before reaching any conclusions about the clinical applications of AMIE.
Structure of AMIE Consultations
AMIE employs an asynchronous multi-agent architecture, allowing distinct processes for dialogue, clinical reasoning, and perception. This design addresses the challenge that a single agent cannot handle natural conversational timing while simultaneously performing intricate reasoning and processing bi-directional audio-visual inputs.
The system comprises three agents:
- Talker Agent: Engages in conversation with the patient, utilizing insights from its counterparts to maintain the dialogue flow.
- Planner Agent: Operates in the background, updating diagnoses and management plans dynamically as the consultation progresses, while also identifying gaps in information and adjusting clinical priorities.
- Perception Agent: Continuously analyzes video and audio streams to detect non-verbal cues and clinical signs, adding contextual relevance to the consultation.
This architecture is essential as deep clinical reasoning necessitates time; thus, patient interactions are not stifled by delays from reasoning or perceptual processes.
Evaluation Against Text and Physicians
The study involved a full evaluation of AMIE’s performance during real-time video consultations, comparing it with a text-only version and evaluations from board-certified primary care physicians using the same video interface. An independent panel of experienced clinicians assessed every consultation based on predefined rubrics.
AMIE demonstrated comparable abilities to the physician group regarding the thoroughness of history-taking, diagnostic accuracy, management appropriateness, and quality of communication. Notably, AMIE’s video performance exceeded that of its text counterpart in physical sign elicitation and guiding actors through examination maneuvers. Patient actors showed a preference for the video format over text, rating it as more user-friendly for discussing health issues.
Automated Pre-Testing and Limitations
Before the human evaluation, Google utilized an automated evaluation suite to refine AMIE’s video system. This involved specific assessments for visual cues, auditory signals, and examination techniques, revealing areas for design improvement prior to actor-based studies. However, Google recognizes limitations, such as the inability of actors to fully replicate the variance found in live patient encounters, which could lead to diagnostic misinterpretation.
The evaluations revealed perception and reasoning errors and noted that technical mishaps could disrupt natural conversation flow. AMIE is still considered a prototype, with Google planning subsequent studies to explore its applicability in clinical scenarios with real patients.
The ongoing project aims to transition from clinical use with a text-based version to assessments in real-world virtual healthcare environments—highlighting progress toward becoming more than just a prototype but a viable healthcare solution.
